  3. Certified true copy of Performance Evaluation Rating for the last two (2) rating periods (for government employees only)
  4. Transcript of Records (authenticated), Training Certificates, Photocopy of Civil Service eligibility duly attested by the Civil Service Commission or a photocopy of license/board rating duly attested by the Professional Regulations Commission, and other documents necessary to support education, experience, training, experience, and eligibility requirements. Please bring your original documents for authentication
  5. *Employment Certificate should indicate duties and functions performed
